Our Individual Day Habilitation services are designed to help adults with intellectual and developmental disabilities build essential life skills and engage meaningfully with the world around them.

🌟 What is Day Habilitation (Individual)?

Day Habilitation is a person-centered, goal-based service focused on helping individuals acquire, maintain, or improve their ability to live and participate in the community. It is available through the Community Integration and Habilitation (CIH) Waiver and is tailored to each person’s Person-Centered Individualized Support Plan (PCISP).

Services are offered in non-residential, community-based settings and are coordinated with other necessary supports like physical, occupational, or speech therapy when applicable.

🧩 What Does Day Habilitation Include?

Our day habilitation activities are flexible, engaging, and deeply personalized. We provide one-on-one assistance with:

  • Self-Care and Daily Living Skills
    Including hygiene routines, dressing, meal preparation, and managing personal space.

  • Personal Health and Safety
    Supporting routines that maintain physical wellness, medication awareness, and safety in public spaces.

  • Communication and Socialization
    Enhancing verbal and non-verbal expression, building relationships, and improving interaction skills.

  • Community Inclusion
    Helping individuals confidently navigate and engage with their communities—whether at a store, library, or café.

  • Recreation and Leisure
    Facilitating participation in hobbies, outdoor activities, and creative outlets.

  • Decision-Making and Autonomy
    Promoting independence through informed choices and personal goal-setting.

🧍 Who Is Eligible?

Day Habilitation services are available to individuals 18 years or older who are eligible under the CIH Waiver. Services are customized based on individual needs, preferences, and life goals.

Our interdisciplinary team works closely with:

  • The individual

  • Family members or guardians

  • Therapists and clinicians (if needed)

  • Case managers and support coordinators

  • Trained Day Program staff and DSPs
